Students Interested in Trying Out for a UI Sports Team
Tryout candidates are not permitted to engage in any Supervised Athletic Activity including practice, strength & conditioning, or any other supervised skill assessment until several items have been completed.
- Physical Examination - You are required to have a physical examination clearing you for participation within the last 6 months before you can try out for a sports team at the University of Iowa. Some options include a physical examination completed at UI Sports Medicine (319-384-7070), University of Iowa Student Health Services (319-335-8394), University of Iowa Hospitals and Clinics QuickCare (phone numbers and contact information here), or by a healthcare provider in your hometown. The cost of having this physical will not be covered by the University.
- If you've had a physical within the last 6 months, you have to fill out all of the documents listed below, show proof of a physical signed by your doctor, including documentation of a Sickle Cell Test according to NCAA Guidelines. If you choose not to have the test, you need to have a signed waiver on file.
- Sickle Cell Solubility Test - Required for any student-athlete beginning their initial year of eligibility and student-athletes trying out for a team, including transfers. (Test conducted by institution, unless documentation of previous test is provided to the institution or prospective student-athlete declines the test and signs a written release.)
- PLEASE PROVIDE A COPY OF EACH OF THEIR HEALTH INSURANCE CARDS (Medical, Prescription, Dental and/or Vision). You must demonstrate proof of in-network insurance that covers sports related injuries.
