Students Interested in Trying Out for a UI Sports Team

Tryout candidates are not permitted to engage in any Supervised Athletic Activity including practice, strength & conditioning, or any other supervised skill assessment until several items have been completed.

• Physical Examination - You are required to have a physical examination clearing you for participation within the last 6 months before you can try out for a sports team at the University of Iowa. Some options include a physical examination completed at UI Sports Medicine  (319-384-7070), University of Iowa Student Health Services (319-335-8394), University of Iowa Hospitals and Clinics QuickCare (phone numbers and contact information here), or by a healthcare provider in your hometown. The cost of having this physical will not be covered by the University.
• PLEASE PROVIDE A COPY OF EACH OF THEIR HEALTH INSURANCE CARDS (Medical, Prescription, Dental and/or Vision). You must demonstrate proof of in-network insurance that covers sports related injuries.

Please fill out each of the pdf forms below ON YOUR COMPUTER completely. You will NOT be able to submit the forms electronically. Print off each completed form, sign & date (if needed) and return to us by postal mail or fax to the address listed below as soon as possible to allow us enough time to get your information prepared before your son/daughter's tryout. If a signature is required on a form, please fill out all the needed information on your computer first, print off the form, and then sign & date it. It is important that you keep one copy of all forms for your records.
